How to make a mode that inserts a fully functional to do list with a check Box in an Apple note either on your desktop OS or iOS.

I am trying to create a new mode that automatically inserts a new page for a to do list in an Apple note complete with fully functional checkboxes by each item. Is there any way this can be done and if so what language must be added to the prompt? I have not gotten it to fully work yet.
